MISSION MEDICAL SUPPLY
DURABLE MEDICAL EQUIPMENT COMPANY
The Power Wheelchair Insurance Coverage Process
-
Your primary care physician or specialist must see you in person and make clinical notes of your need for a power or manual chair. This is called a "Face to Face". (Physicians can use this Face-to-Face Evaluation form)
-
Once Mission Medical Supply receives the face-to-face evaluation report, you will need to schedule an evaluation by a Physical Therapist for seating evaluation as well. This is often done in the comfort of your home. (PT can use the seating mobility eval. below).
-
Once that Evaluation has been done, it is submitted to the Physician who performed the Face to Face
-
Once that Physician has signed off on the evaluation, Mission Medical Supply will submit a specific document called a Seven Element Order. Per insurance guidelines, the Seven Element Order and Evaluation must be sent separately, signed, and dated.
-
Once the Seven Element Order is signed Detailed Product Description (DPD) must be signed. Again, according to insurance, this sequence must be followed.
-
Once the Detailed Product Description (DPD) is signed, Mission Medical Supply will submit the claim to Medicare / Insurance to authorize or deny the wheelchair.
-
Normally Medicare or Insurance will take up to 3 weeks to determine medical necessity and ensure everything is in order.
-
Once Mission Medical Supply has approval, we will order the Power Wheel Cha which takes 1-2 weeks to get delivered.
